Partners in Recovery, LLC (including Partners Behavioral Healthcare)
is required by law to protect your health information, provide this notice, and
follow the privacy practices described here.
How We May Use and Share Information
- Treatment: Share information with your care team to coordinate services.
- Payment: Use information to bill you or your insurance company.
- Healthcare Operations: Improve services, train staff, and run programs.
- As Required by Law: Public health, legal orders, safety emergencies, etc.
When We Need Your Permission
We will ask for written authorization before:
- Sharing clinical notes (except when allowed by law).
- Using your information for marketing or fundraising.
- Any other use not listed in this notice.
You can revoke your permission at any time in writing.
No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
Your Rights
- See and get a copy of your medical records.
- Request corrections if information is wrong or incomplete.
- Ask us to limit how your information is used or shared.
- Request confidential communication methods.
- Get a list of disclosures (except for treatment, payment, operations).
- Receive a paper copy of this notice.
- File a complaint without fear of retaliation.
Our Responsibilities
We must maintain the privacy and security of your health information, notify you
if a breach occurs, and follow the practices described in this notice. We will not
use or share your information other than as described here unless you authorize us in writing.
