Request a time: Fill out the online form with your preferred date and time for the comprehensive assessment.
Complete all required information: In order to process your request we must have all the required information on the form completed.
Submit your request: Once you’ve filled out the form, hit the submit button.
Confirmation: Once submitted, our dedicated admissions team promptly reviews the request and reaches out to confirm the appointment.
