TELEHEALTH OUTPATIENT PROGRAM

Telehealth: Quality care from the comfort of your home.

Partners offers individuals with substance or alcohol use a convenient way to access treatment services. With the addition of telehealth programming, we are expanding the number of people was can help. Our clients can trust that the curriculum is the same and our commitment to clinical excellence has not wavered.

Telehealth services are open to any Minnesota resident 18 years of age or older. Rural populations can use telehealth to overcome shortages of specialized behavioral healthcare and resources. It is also a convenient solution for individuals with transportation limitations or lack of childcare.

Telehealth is offered through a confidential, HIPAA-compliant video conferencing service. Patients will receive a personalized access link to be used throughout the course of treatment from the comfort of home. Programming is offered 3 nights a week (Tuesdays, Wednesdays and Thursdays) from 6 pm to 9 pm. Your individual chemical health assessment will assist in determining how many days per week you attend.

Step 1. We are here to listen.

Call 888-648-7652. Our Admissions Representatives are available to speak with you. Many of our staff have personal experience of recovery and can empathize with what you are going through.

We will gather information and help you navigate the process- including scheduling an assessment or sending us your referral and obtaining approval from insurance or public funding.

Step 2. Complete an assessment.

During your assessment, we will develop an understanding of your needs. If you have already completed a chemical health assessment through another provider, you can simply fax or email us this information.

Step 3. Receive personalized treatment recommendations.

Based on your individual circumstances and clinical needs, you will receive a determination of which treatment options are the best fit for you. We will review it together and will answer any questions you may have.

Step 4. Begin.

Once you have reviewed your recommendations and have agreed upon it, it’s time to begin. We will schedule a date for you to begin, discuss any additional assistance you may need, and get started.
